The month of May blossoms annually with commencement ceremonies honoring students graduating from high schools, colleges and universities. For each graduate, the occasion represents a major milestone along their road to adulthood. It's also an appropriate time to assess their dental development.
Although our teeth and gums continue to change as we age, the greatest change occurs during the first two decades of life. In that time, humans gain one set of teeth, lose it, and then gain another in relatively rapid succession. The new permanent teeth continue to mature, as do the jaws, up through the time many are graduating from college.
Of course, you don't have to be in the process of receiving a diploma to “graduate” from adolescent to adult. If you are in that season, here are a few things regarding your dental health that may deserve your attention.
Wisdom teeth. According to folklore, the back third molars are called wisdom teeth because they usually erupt during the transition from a “learning” child to a “wise” adult. Folklore aside, though, wisdom teeth are often a source for dental problems: The last to come in (typically between ages 17 and 25), wisdom teeth often erupt out of alignment in an already crowded jaw, or are impacted and remain hidden below the gums. To avoid the cascade of problems these issues can cause, it may be necessary to remove the teeth.
Permanent restorations. Though not as often as in adults, children and teens can lose teeth to disease, injury or deliberate removal. Because the jaw is still in development, dental implants are not generally advisable. Instead, patients under twenty often have temporary restorations like partial dentures or bonded bridges. As the jaws reach full maturity in a young adult's early 20s, it's often a good time to consider a permanent implant restoration.
Smile makeovers. An upcoming graduation is also a great reason to consider cosmetic smile upgrades. When it comes to improving a smile, the sky's the limit—from professional teeth whitening for dull teeth to porcelain veneers or crowns to mask dental imperfections. It's also not too late to consider orthodontics: Braces or the increasingly popular clear aligners can straighten almost anyone's teeth at any age, as long as the person is in reasonably good health.
This may also be a good time to update your own personal care. Regular dental visits, along with daily brushing and flossing, are the foundation stones for keeping your teeth and gums healthy throughout your life. So, as you “commence” with this new chapter in your life, make a dental appointment now to “commence” with a renewed commitment to your dental health.

It can be alarming to be awakened in the middle of the night by a screeching, gritting sound coming from your child’s bedroom. No, it’s not a scene from a horror movie: it’s your child grinding their teeth as they sleep — a behavior so prevalent in children under eleven it’s considered normal.
That doesn’t mean, however, you should completely ignore it. While it isn’t harmful for most children, a few can encounter tooth wear, pain or trouble sleeping that calls for some form of intervention.
The causes for tooth grinding and similar habits known collectively as bruxism aren’t thoroughly understood, but in children it’s believed linked to the immaturity of the neuromuscular system that controls chewing. Some point to shifts from one stage of sleep to another — more than 80% of grinding episodes occur in lighter stages of sleep and only 5% to 10% during the deeper Rapid-Eye-Movement (REM) stage. It also seems prevalent in children who snore or have other symptoms of sleep apnea.
One primary concern is how the behavior can affect teeth, particularly through abnormal wear. The teeth, of course, make hundreds of contacts with each other every day during eating, speaking or jaw movement. If, however, the forces generated during these contacts chronically exceed normal parameters, as with bruxism, it can cause accelerated tooth wear. This can result in a higher susceptibility to tooth decay and appearance changes later in life.
If your child is exhibiting problems associated with teeth grinding, there are ways to address it. We may recommend a thin, plastic mouthguard they wear while sleeping that prevents the teeth from making solid contact with each other. We may also refer you to an ear, nose and throat specialist if we suspect signs of sleep apnea. And, children under severe psychological stress, which can also trigger teeth grinding, could benefit from behavioral therapy.
The good news is most grinding habits fade as children enter their teens. In the meantime, keep a watchful eye and see us if you notice any indications this common habit is affecting their health and well-being.

Great oral health takes work
Healthy smiles don't just happen—they take diligent care at home. Key to smile longevity proves to be these careful preventive practices:
- Brush your teeth two times a day for two minutes, as the American Dental Association (ADA) advises children through senior adults. Use a fluoride toothpaste and soft brush, both protect the integrity of your tooth enamel. Be sure to gently brush your gums, tongue, and the roof of your mouth, as well.
- Floss around all your teeth every day—before bedtime preferably. Flossing removes plaque and its dangerous bacteria.
- Eat a diet high in fiber, calcium, and low-fat protein. Drink water. Avoid processed sugars.
- Wear a mouth guard during any athletic activity.
- Wear a bite guard if you consistently grind/clench your teeth. This habit is called bruxism, and it damages your tooth enamel and dental bite, while also causing jaw joint problems.
- Look into your mouth to observe any changes in your tooth color or shape or in the texture of your soft tissues.
- Never ignore dental sensitivity or pain. Call Dr. Rollins immediately.
- Keep your dental appointments. We all need six-month hygienic cleanings and examinations to remove plaque and to detect problems early.
- Stop all tobacco usage. Smoking causes bad breath, oral cancer, and periodontal disease. Also, tobacco impairs the ability to heal after procedures, such as extractions and tooth replacement.

People who’ve lost all their teeth (a condition known as edentulism) face a decision on how to restore their lost function and appearance. And there are a number of options to consider.
A fixed bridge supported by dental implants, for example, is a good choice for patients who still have sufficient bone structure in their jaw. It’s not a good choice, however, for those with the opposite situation — who’ve experienced significant bone loss which has also affected their facial structure. For them, there’s a better alternative that also uses implants for support — the overdenture.
An overdenture is similar to a traditional denture, in that it’s made of life-like crowns permanently set in denture plastic, and may either partially or fully cover the roof of the mouth. The main difference, though, is that unlike traditional dentures which rest for support on the gum ridges, an overdenture is supported by strategically placed implants that the denture fits over and connects to — hence the name “overdenture.”
There are a number of advantages for an overdenture, especially for patients with bone loss. A removable, implant-supported denture can be designed to replace lost tissues that have altered facial appearance — to “fill in” the face and restore aesthetic harmony. Patients who’ve previously worn dentures will also often find their speech better improved than with fixed bridgework.
Because it’s removable, an overdenture and the underlying gums are easier to clean, which helps inhibit disease and lessen further bone loss. It also allows you to properly care for the denture, which can extend its longevity and reduce future potential maintenance and replacement costs.
If you would like to consider removable overdentures as an option, you should begin first with a thorough oral exam that includes evaluating the status of your bone, jaw and facial structure. From there we can advise you if overdentures are the best choice for you.
